今年改訂されたDLBの新国際診断基準には心交感神経シンチグラフィとドパミントランスポーターイメージング、脳血流・代謝検査と、3つもの核医学検査が取り入れられている。新たに取り入れられたCingulate Island Sign(CIS)はFDG-PETでの後部帯状回の相対的代謝保持を示すが、関心領域を最適化した検討により、脳血流SPECTでもADとDLBの判別において良好な結果が得られている。このCISを中心に核医学検査の有用性について述べる。
The Dementia with Lewy Bodies (DLB) Consortium has refined its recommendations about the clinical diagnosis of DLB this year. Three nuclear medicine procedures are included in the new recommendation; 123iodine-metaiodobenzylguanidine (MIBG) myocardial scintigraphy, dopamine transporter (DAT) imaging, generalized low uptake on SPECT/PET perfusion/metabolism scan with reduced occipital activity ± the cingulate island sign (CIS) on FDG-PET imaging. Recently we researched about CIS in brain perfusion SPECT and concluded that Optimized CISs using brain perfusion SPECT are clinically useful for differentiating DLB from AD. This feasibility of nuclear medicine in diagnosing DLB is described in this article.
