悪性腫瘍に対する治療は年々進化し続け、近年はそれぞれの悪性腫瘍における様々な因子によって治療方針を選択する個別化医療が広まってきている。これらの因子の中で腫瘍低酸素は、特に放射線治療において重要な因子として近年注目されている。本稿ではPETを用いた腫瘍低酸素分子イメージングとこれをコンパニオン診断として使用した治療への応用に向けての展望を紹介する。
The individualized medicine by various tumor factors is now spreading. Tumor hypoxia is a major factor of prognosis prediction. Positron Emission Tomography (PET) is a useful modality to evaluate tumor hypoxia noninvasively. The current topic is how to arrange the therapy who has tumor hypoxia using hypoxia PET as a companion diagnostic. Dose escalation of radiotherapy is the most promising therapy. Whereas, proton beam therapy, heavy particle beam therapy, or combination of chemotherapy including platinum agent may be also available for the hypoxic tumor. We need more investigations to find better therapeutic strategies for the patient with hypoxic tumor.
