負荷心筋血流MRIは心筋虚血の正確な診断法で予後評価にも有用である。近年、薬剤負荷の効果判定に脾臓の造影効果が利用されている。また、負荷心筋血流MRIは冠動脈疾患治療方針決定においてFFRと同等の有用性が示唆されている。圧縮センシングによる画質向上や客観性・再現性の高い心筋血流定量解析は今後普及が期待される。
Stress perfusion cardiovascular magnetic resonance (CMR) can provide excellent diagnostic performance for the detection of coronary artery disease (CAD) and high prognostic value in patients with suspected CAD. Splenic switch-off is a novel sign for the assessment of stress adequacy during adenosine stress perfusion CMR. Recent study suggested that stress CMR guided management of patients with stable angina might be noninferior compared with invasive fractional flow reserve (FFR). Fully quantitative perfusion CMR may permit more objective and reproducible assessment ischemia in CAD patients. Compressed sensing is an emerging technique having a great potential to improve the perfusion CMR image quality.
