Evaluation of Body Temperature during Pancreaticoduodenectomy for Pancreatic Cancer in Elderly Patients Aged over 75 Years Old
Summary
Background: Body temperatures in elderly patients undergoing a pancreaticoduodenectomy(PD)for treatment of pancreatic cancer have not been fully evaluated. We aimed to compare body temperature during a PD between elderly(75 years old)and non-elderly patients. Methods: This retrospective study included patients who underwent PD between April 2012 and April 2014 at the Kanagawa Cancer Center. Patients were categorized into 2 groups: elderly patients(≥75 years of age: group A)and non-elderly patients(<75 years of age: group B). Body temperatures were compared between the groups. Results: We evaluated 58 patients-14 were classified into group A, and 44 into group B. Preoperative clinicopathological outcomes demonstrated no significant differences between patients. A comparison of body temperatures between the groups showed 36.3℃(before surgery)/36.1℃(at 1 hr)/36.3℃(at 2 hr)/36.7℃(at 3 hr)/36.8℃(at 4 hr)/37.1℃(at 5 hr)/37.4℃(at 6 hr)in group A compared to 36.2℃(before surgery)/36.2℃(at 1 hr)/36.4℃(at 2 hr)/36.6℃(at 3 hr)/36.9℃(at 4 hr)/37.0℃(at 5 hr)/37.3℃(at 6 hr)in group B. Results did not significantly differ between the groups. Conclusions: Our study suggests that body temperature is not affected by a patient's age.

背景: 手術中の体温管理は術後の早期回復に影響する重要な因子である。しかし,膵頭十二指腸切除(pancreaticoduodenectomy: PD)症例での体温変化に対する年齢因子の影響は不明である。今回,75歳以上のPD症例における全身麻酔導入後の膀胱温度変化を,75歳未満の非高齢者のPD症例と比較検討した。対象と方法: 2012年4月～2014年4月までの期間に神奈川県立がんセンター消化器外科において膵癌に対して70例のPDが施行され,このうち膀胱温度変化が測定できた58例を対象とした。膀胱温度変化は,全身麻酔導入後から6時間にわたって検討した。結果: 75歳以上の高齢者群(高齢群14例)と75歳未満の非高齢者(非高齢群44例)の背景因子を比較すると,性別・術前のASA-PS・体重・BMI・術前併存疾患の有無・手術時間・出血量に差はなかった。膀胱温度変化は高齢群で36.3/36.1/36.3/36.7/36.8/37.1/37.4℃,一方で非高齢群では36.2/36.2/36.4/36.6/36.9/37.0/37.3℃であり,年齢に伴う膀胱温度変化に統計学的な有意差はみられなかった。結論: 今回の検討では,高齢群は非高齢群と比較して術中体温変化に年齢の影響はみられなかった。75歳以上の高齢者においても,四肢の末端部分を保温することで体温保持は可能と考えられた。
