Summary
Japan is facing an aging society. As the number of elderly people increases, the number of elderly cancer patients is increasing. In-home medical care and long-term care need to be deepened to support elderly cancer patients. For this reason, municipalities have implemented the project to promote coordination between in-home medical care and long-term care. All prefectures have a role to support the project. In Okinawa Prefecture, the Prefectural Medical Association has been entrusted with the implementation of the support project. The contents of the support project are listed below.(1)Liaison meeting in which municipal staff, coordinators entrusted to the medical association, and prefecture staff participate.(2)Creating tools to support in-home medical care and long-term care coordination.(3)Hold workshops to learn examples of initiatives in pioneering regions. We have provided support for the promotion of in-home medical care and long-term care coordination across municipalities. This report outlines the in-home medical care and long-term care promotion support project, taking the case of Okinawa Prefecture as an example, and discusses the in-home medical care and long-term care coordination required for cancer patients.

高齢化社会を迎えている本邦において,高齢化を背景としてがん患者数は増加している。高齢のがん患者を支えるために在宅医療と介護は連携を深める必要があり,各市町村において在宅医療・介護連携推進事業が実施されている。都道府県にはその事業を支援する役割があり,市町村の担当者や地区医師会に委託されているコーディネーターを対象とした連絡会議の開催,在宅医療・介護連携支援ツールの作成,先進地域の取組事例などを学ぶ研修会の開催など,広域的な在宅医療・介護連携の推進に向けた支援を実施してきた。在宅医療・介護連携推進支援事業について沖縄県の取り組みを例に概説し,がん患者に必要な在宅医療・介護連携について考察する。
